COVID-19 Vaccination Programs in Chicago and the Metropolitan Area
Updated February 16, 2021 | The State of Illinois is working with local health departments and providers across the state to provide COVID-19 vaccinations. Eligible persons as outlined in Phase 1a or 1b -- such as health care workers, first responders, essential workers, or anyone 65 years of age and older -- can register for vaccinations. Chicago is in phase 1B of COVID-19 vaccinations: Chicagoans age 65+ and specific groups of highest-risk and front-line essential workers will be eligible for vaccination. There are three ways Chicago residents can receive a COVID-19 vaccine.

There are three ways Chicago residents can receive a COVID-19 vaccine.
- Your health care provider. Your first contact should be your health care provider, including your primary care provider, health clinic, or hospital where you have gotten medical care in the past.
- Your pharmacy. The City of Chicago has provided vaccines to pharmacies across Chicago.
- Your employer. Specific employers in prioritized essential worker groups will be able to distribute the COVID-19 vaccine to their employees in 1b.

Cook County Government, Cook County Health and Cook County Department of Public Health are pleased to offer COVID-19 vaccinations to those living or working in Cook County. On this site, you can sign up for COVID-19 updates and information to schedule an appointment when administration expands to your phase. Providing all the information requested will ensure that you are notified properly as the program expands. You also can call the Vaccine Signup Hotline at 833-308-1988; information is available in English, Spanish, and Polish.
CCDPH Organization On-Site Vaccination Sign-Up Request
Community organizations, such as adult daycare, community centers, employers, senior housing, shelters, places of worship, or other large institutions, can also sign up to organize on-site vaccinations. To qualify for mobile team assistance you must:
1. Employ or serve individuals in Phase 1b with limited resources or access to care within suburban Cook County (excluding Evanston, Skokie, Oak Park, or Stickney Township).
2. Have a secure space to conduct vaccination while maintaining social distancing
3. Support clinics for dose 1 and 2
4. Support setting up appointments and pre-register staff/clients

MCDH has a plan in place to vaccinate McHenry County priority groups and residents and is following the Advisory Committee of Immunization Practices (ACIP) and CDC’s recommendation and distributing the first batches of vaccines that we receive to healthcare workers and long-term care facilities. Because supplies of vaccines will be limited until production ramps up, it could take months before we are able to begin providing a vaccine to other prioritized groups.
